How to Get Insurance to Cover TRT
Insurance requires a documented diagnosis of hypogonadism. What you need:
- Two morning testosterone blood draws (before 10 AM) showing levels below reference range
- Symptoms documented in your medical record: fatigue, low libido, mood changes
- Diagnosis code: ICD-10 E29.1 (primary) or E23.0 (secondary hypogonadism)
- Generic testosterone cypionate is almost always covered. Gels, pellets, and compounded formulations often aren't.
If Insurance Won't Cover It
